100/80/50 coinsurance level or less
Endodontics & Periodontics must be in major services
$ 25.00 minimum deductible for basic and major
Lifetime deductible available
Maximum calendar year benefit is $2,000

5-9 Lives– Basic Benefits
100/90/60/50 coinsurance level or less
Orthodontia coverage available for 5+ enrolled
Endodontics & Periodontics can be in basic or major services
$ 25.00 minimum deductible for basic and major
Lifetime deductibles are available
Maximum calendar year benefit is $2,000

10+ Lives– Basic Benefits
100/100/60/50 is the maximum coinsurance levels available
Endodontics & Periodontics can be quoted in basic and major
Minimum deductible is $0
Lifetime deductible credit available
Maximum calendar year benefit is $ 2,500
Ortho Lifetime Maximum benefit is $ 2,500

Healthy Choices offers Short and Long-Term Disability coverage to fill this important need. Employees need to insure their paychecks and we make it easy to provide the right coverage for them.

Elimination Periods for Accident 1st or 8th day
Elimination Periods for Sickness 8th day
Maximum Benefits 60% of insured weekly earnings up to $1,500 per week
Benefit Duration Choices 13 or 26 weeks

Long-Term Disability

Elimination Periods for Accident and Sickness 90 or 180 days
Maximum Benefits 60% of insured monthly earnings up to $6,000 per month
